If you're interested in working with Kim, complete the form with a few details about what you would like Kim’s help with. We regularly evaluate the capacity to take on new patients. We will keep your form on file and contact you once there are new patient appointments available. We respond to requests as appointment availability permits and in the order received.

Wait time for scheduling an appointment can be up to 1-1.5 months. Appointments are typically offered 30 -45 days in advance (if we contact you to schedule, the available appointment is likely within 30 -45 days). We generally do not have the capacity to offer new patient intake appointments for acute issues.

If you are returning patient, please text the clinic phone at 360-927-5375 to get back on the schedule.

Please only submit the form one time. Your submission is time stamped and keeps your place on the wait list to be contacted for scheduling. Multiple submissions will have the most RECENT submission date next to your name and will make the wait time LONGER.

Once you complete the form and press submit, you will see a white confirmation screen that your appointment request was successfully submitted.
