INTRODUCTION
All content supplied by Sea2Summit Physical Therapy and Sports Rehabilitation LLC DBA Kim Burns, DPT OCS Physical Therapy and Sports Rehabilitation are subject to the following express terms. And these terms “We”/ “Us” / “Our” means Sea2Summit physical therapy and sports rehabilitation LLC DBA Kim Burns, DPT OCS physical therapy and sports rehabilitation and “you”/“your” / “yours” means the direct client as well as the indirect client choosing to engage with the online content published published by Sea2Summit physical therapy and sports rehabilitation LLC DBA Kim Burns, DPT OCS physical therapy and sports rehabilitation.

GENERAL

All online content is intended for general and informational use only. It is not a substitute for a customized corrective exercise program based on your individualized movement exam data that is collected in a face to face physical therapy examination. Please use at your own risk. 

This practice is paperless and only supplies intake paperwork and customized corrective exercise prescriptions via electronic means ie email, text messages, or link through the secure online patient portal messaging system.

This is a small solo practice and due to space limitations, patients may not sit in the waiting area more than 5-7 minutes prior to or after their scheduled appointment time due to privacy concerns to the patient scheduled before or after their appointment. Phones are to be on silent while in the clinic space. Please save phone conversations for before or after your PT session. Please keep conversations in the clinic waiting area in a low or whisper voice so as not to disturb the other sessions occurring while you are waiting. Every effort is made to hold appointments on time. 

Scheduling of new patient appointments may be completed online at https://www.tebra.com/care/provider/kimberly-burns-pt-1700121829#about. Current patients may schedule via text messaging the main office line at 360-927-5375 or messaging through the secure online patient portal. While every effort is made to respond to all needs in a timely manner, please allow 1-2 business days for a response to a voicemail or email. Text messages are generally answered in the afternoon, and at the end of usual business hours. While every effort is made to address, your individual needs, it is not possible to hold extended conversations over the phone or with unannounced visits. Please save your questions or details about your specific individual issue for your scheduled Initial evaluation appointment. Evaluations and progress assessments will be performed 1:1 by Kim Burns, DPT OCS. Subsequent follow up visits related to completing the plan of care outlined by Kim Burns, DPT OCS will be completed by a licensed physical therapy assistant who has been directly trained and demonstrates competence in the treatment protocols, manual therapy techniques, and condition monitoring by Kim Burns, DPT OCS. Patients may have up to four visits with an assistant before being reassessed directly by Kim Burns, DPT OCS in accordance with state law. All treatment sessions are supervised by Kim Burns, DPT OCS.

We do not offer childcare at Sea2Summit Physical Therapy and Sports Rehabilitation LLC DBA Kim Burns, DPT OCS Physical Therapy and Sports Rehabilitation nor can we allow any unsupervised minors to wait in the waiting area during appointment sessions. Sea2Summit Physical Therapy and Sports Rehabilitation LLC DBA Kim Burns, DPT OCS Physical Therapy and Sports Rehabilitation is not responsible for the safekeeping of your personal belongings. Please bring only what you need and keep your belongings in the treatment room with you during the appointment session.

When engaging with content published online, you agree that you have been examined by a qualified medical professional, have been found good physical health and physically able to perform fitness related activities. It is your responsibility to consider previous injury or conditions that may impact your ability to engage in physical activity for fitness. The online published content is there to help and to inform but it is not possible diagnose causes of injuries or illnesses without a direct assessment in a physical therapy evaluation. Your qualified medical practitioner and your intuition are the first lines of defense with an injury. You voluntarily participate in using published online content with full knowledge that there is a risk of personal injury, property loss or death. In using the published online content, you agree that neither you your heirs, assigns, or legal representatives will sue or make any other claims of any kind whatsoever against Kim Burns, DPT OCS, Sea2Summit Physical Therapy and Sports Rehabilitation LLC DBA Kim Burns, DPT OCS Physical Therapy and Sports Rehabilitation whether caused by negligence or otherwise.

If a patient is:

- obviously under the influence of psychoactive substances

- visibly unwell, has a fever of more than 99.5, uncontrollable cough, egregiously runny nose, a positive COVID test or COVID exposure less than 5 days ago

-Engaging in disruptive behavior (ie engaging in loud conversations In the waiting area, using an electronic device unmuted, using profanity or vulgar language)

- Engaging in unconsented physical contact with staff or other patients 

- is more than 5 minutes past the scheduled appointment time  
Sea2Summit Physical Therapy and Sports Rehabilitation LLC DBA Kim Burns, DPT OCS Physical Therapy and Sports Rehabilitation reserves the right to refuse entry, discharge or reschedule a patient from the practice at any time with or without notice.


This is a small solo practice and due to scheduling limitations and consideration for others who also wish to be treated at Sea2Summit Physical Therapy and Sports Rehabilitation LLC DBA Kim Burns, DPT OCS Physical Therapy and Sports Rehabilitation, please give 24 hours or more notice for appointment cancellations or rescheduling.  More than 3 last minute cancels or no shows will result in discharge from care with or without notice. 

Physical therapy treatment sessions involve physical activity and movement. You may dress to your own needs and comfort. Simple, breathable clothing that you can move freely in. If you tend to be on the sweatier side, please bring a sufficient towel. You are always welcome to bring your own yoga mat or other personal equipment that you use in home if you prefer. Any mats, equipment weights or bands in the clinic are not for sale and are our clinic property. If you enjoy using the equipment available during your physical therapy sessions feel free to ask where the equipment was purchased and obtain your own for your own personal use.


Please allow 30 to 60 days for insurance claims to process. Co-pays are due at the time of service. Statements for balances are sent out monthly and are due upon receipt. Past due balances of over six months will be sent to collections. Patients with past due balances will not be rescheduled until the balance is paid in full or a payment plan with an initial payment has been completed. Please contact the customer service number on the back of your insurance card for questions regarding your individual physical therapy benefit coverage. It is your responsibility to know your own healthcare benefits.
